At Pantheavalley, we believe everyone deserves a home they feel proud of. Our supported living service is about far more than accommodation. It is about providing thoughtful, person centred support that empowers people to live independently, confidently and safely within their own community.
Healing with hearts, supporting lives is not just a phrase for us, it reflects how we work every day, with compassion, respect and genuine commitment.
Supported living enables adults to live in their own home while receiving the support they
need. Individuals usually hold their own tenancy and remain in control of their daily routines, finances and personal decisions.
Support is flexible and tailored, helping people to:
• Maintain independence
• Develop life skills
• Make everyday choices
• Build confidence
• Achieve personal goals
Our focus is always on encouraging independence while providing reassurance and practical
assistance where needed.
Pantheavalley provides supported living for young adults, adults and seniors, including
individuals living with:
• Learning disabilities
• Autism
• Physical disabilities
• Acquired brain injuries
• Mental health needs
• Complex conditions
We design support around the person, not the condition, ensuring dignity, choice and control
remain central.
Every support plan is created after carefully listening to the individual, their family and
relevant professionals. We take time to understand their background, strengths, aspirations
and support requirements.
Â
Support may include:
• Personal care and daily living assistance
• Medication support
• Budgeting and money management
• Cooking and meal planning
• Education, volunteering or employment support
• Access to community activities
• Emotional wellbeing support
Â
Support plans are regularly reviewed to celebrate progress and set new goals.
We understand that moving into supported living can feel like a significant step. Whether
transitioning from residential care, the family home or children’s services, our team provides
consistent guidance and reassurance.
We work closely with families and professionals to ensure the transition is smooth, positive
and well planned, helping each person feel secure and confident in their new home.
Support can be arranged through:
• Local Authority funding following a Care Act assessment
• NHS funding, including Continuing Healthcare where applicable
• Private funding for individuals and families arranging care directly
If you are unsure which route applies, our team is happy to guide you.
Once a referral is received, we arrange a detailed assessment to understand the individual’s needs, preferences, strengths and long term goals. We work closely with social workers, healthcare professionals and families during this stage.
We identify suitable accommodation options based on the person’s support needs, preferred location and lifestyle choices. Viewings can be arranged to ensure the right environment is selected.
We develop a personalised support plan and, where required, submit a detailed funding proposal to the Local Authority or NHS for approval. Private clients receive a clear cost breakdown and service agreement.
Once funding is confirmed, either through the Local Authority, NHS or private arrangement, we finalise the placement details and tenancy arrangements.
We agree a move in date and provide full transition support. Our team ensures the individual feels safe, prepared and confident as they settle into their new home.
Feeling safe, comfortable and valued is essential. We work with housing providers and
landlords to secure suitable accommodation, including:
Â
• Self contained apartments
• Shared houses
• Adapted properties
• Homes within local communities
Â
Where individuals choose shared living, we carefully consider compatibility to promote
positive and supportive living arrangements.
• Person centred care
• Experienced and trained support staff
• Clear communication with families and professionals
• Strong focus on independence
• Compassionate, respectful approach
Â
At Pantheavalley, we are committed to helping people live meaningful, fulfilling lives in
homes they truly call their own.
We are a trusted care provider committed to delivering compassionate, person centred support across supported living, residential care and domiciliary care services. From personalised home care to 24 hour residential support and structured supported living, our goal is to help individuals live with dignity, independence and confidence in safe and
nurturing environments.
