their own homes. It is suitable for adults who require assistance with daily living but wish to
remain in familiar surroundings.
Support can range from short visits each week to multiple visits per day, depending on
assessed needs.
Our focus is to maintain independence while ensuring safety, comfort and emotional
wellbeing.
Pantheavalley provides domiciliary care for young adults, adults and seniors and over,
including individuals living with:

• Mental health conditions

• Complex health conditions
Every care plan is tailored to the individual, recognising personal preferences, routines and
long term goals.
Our approach is built on partnership. We work closely with individuals, families, social
workers and healthcare professionals to ensure care is coordinated and responsive.

Support may be funded through the Local Authority following a Care Act assessment, through NHS funding where eligible, or arranged privately by individuals and families.
We carry out a comprehensive assessment to understand care needs, health requirements and personal goals.
A personalised care plan is developed outlining the level and type of support required.
Once funding or private arrangements are confirmed, we agree a start date and introduce the care team.
We provide consistent, reliable support and regularly review care plans to ensure quality and progress.
