What is supported living?

Supported living enables adults to live in their own home while receiving the support they
need. Individuals usually hold their own tenancy and remain in control of their daily routines, finances and personal decisions.

Our Process

Referral and Funding Pathway

Support can be arranged through:

• Local Authority funding following a Care Act assessment
• NHS funding, including Continuing Healthcare where applicable
• Private funding for individuals and families arranging care directly

Initial Assessment

Once a referral is received, we arrange a detailed assessment to understand the individual’s needs, preferences, strengths and long term goals. We work closely with social workers, healthcare professionals and families during this stage.

Accommodation Matching

We identify suitable accommodation options based on the person’s support needs, preferred location and lifestyle choices. Viewings can be arranged to ensure the right environment is selected.

Support Planning and Proposal

We develop a personalised support plan and, where required, submit a detailed funding proposal to the Local Authority or NHS for approval. Private clients receive a clear cost breakdown and service agreement.

Funding Approval and Agreement

Once funding is confirmed, either through the Local Authority, NHS or private arrangement, we finalise the placement details and tenancy arrangements.

Transition and Move In

We agree a move in date and provide full transition support. Our team ensures the individual feels safe, prepared and confident as they settle into their new home.

Feeling safe, comfortable and valued is essential. We work with housing providers and
landlords to secure suitable accommodation, including:

• Shared houses
• Adapted properties
• Homes within local communities
